Talbot Hospice is here for more than just “end-of-life” care. We’re here to help you improve the quality of life of your patients.

We share your commitment. Talbot Hospice is here to help physicians and healthcare providers achieve improved outcomes for patients facing serious illness. When you’re just not comfortable having these tough conversations, we can either train you and your team, or talk to your patient for you.

We’re here to provide supportive care for families and coordinated care for patients.
The benefits of Hospice don’t stop there. Research shows that hospice care reduces ER visits as well as improving comfort and quality of life during terminal illness.

Hospice interdisciplinary teams collaborate with you to provide the compassionate and comprehensive care, spiritual and psycho-social support, personal care and bereavement services. In addition to our core programs below, we also provide specialized care for veterans and children. 

 

Talbot Hospice Core Programs

The goal of Talbot Palliative Care is to improve the quality of life for an individual at any stage in a serious illness, whether the disease is curable, chronic, or life-limiting. Patients receiving palliative care may still continue curative treatment. Talbot Palliative Care works with a patient's physician to manage symptoms and help maintain independence, reduce repeated hospitalizations and ER visits. Talbot Palliative care requires a referral from Community physician, such as primary physician, cardiologist, oncologist, etc.
Hospice Care

Hospice care provides compassionate care and promotes dignity and comfort at the end of life. Hospice care is for individuals who have ceased curative treatments and /or exhausted all medical treatments. Hospice care focuses on managing pain, symptoms, and stress of end-stage, serious illness during the terminal phase. The hospice care team cares for the "whole person" and addresses the physical, emotional, social, and spiritual needs of both the patient and their loved ones. Hospice care requires a physician referral and signed attestation of a terminal prognosis of 6 months or less. 

Bereavement & Grief Support

Talbot Hospice offers support to individuals and families before and after the death of a loved one. The bereavement team offers individual and family consultations for adults and children, as well as a number of support groups that meet on a regular basis.  Bereavement services are offered without charge and there is no requirement for the deceased loved one to have been a Talbot Hospice patient. Common Questions

Can I bill Medicare or commercial insurance for services once my patients are referred to Hospice?

Yes, you can. Primary attending physicians continue to bill Medicare Part B and other carriers for the patient’s regular office visits or for all covered services. New billing codes are also available for care plan oversight, so reimbursement can be obtained for telephone calls to nurses, as well as patients.

Do commercial insurance plans have hospice benefits?

Most third-party payers now have a hospice benefit, similar to the Hospice Medicare Benefit.
