We are very excited about an ongoing educational initiative that Talbot Hospice is spearheading to promote the importance of advance care planning. The project is called Talbot Conversations: Make Your End-of-Life Wishes Known. The goal of our project is to encourage as many people as possible to understand their choices at the end of life, to complete their advance directive paperwork and to share their wishes with their loves ones and physician. We are proudly partnering with The Conversation Project, a national initiative dedicated to helping individuals discuss, document and share their wishes for end-of-life care.
The Talbot Conversations initiative sprang from our study of the book The Conversation: A Revolutionary Plan for End-of-Life Care by Harvard physician Angelo Volandes, MD, and from the inspiring NPR story out of La Crosse, Wisconsin where 96% of residents who die have an advance directive. You may be wondering why this topic relates to you. Studies show that 90% of Americans believe it is important to have these discussions with their families, yet less than 30% have done so. The consequences can be devastating, particularly when family members in the throes of emotional distress must make decisions while guessing what a relative would have wanted.
To help you participate in Talbot Conversations: Make Your End-of-Life Wishes Known we have provided materials for your review: 1) a pledge form to sign to return to us to indicate your intent to participate, 2) The Starter Kit provided by The Conversation Project to help you think about your wishes, and 3) Advance Directive paperwork.
Talbot Hospice will be providing ongoing educational sessions to help you complete the forms every second Tuesday of the month and can also bring the talk to your group, business or organization wherever you meet. Please call in advance to register 410-822-6681.
