Pathways Support for the Caregiver

Our Pathways Program provides a range of support for caregivers and includes non-medical holistic services, education and advocacy for people with a limited life expectancy. The Director of Pathways helps guide patients and families in identifying needs. All Pathways services are offered without charge and are fully funded by donations to Talbot Hospice.

Our services include:

  • The Director of Pathways can help guide patients and families and identify needs
  • Volunteers are available for respite care for the caregiver and companionship for the patient, transportation, emotional support, and comfort therapies
  • Families and caregivers are invited to our weekly Caregiver Support Group at Hospice House
  • Caregiver coaching, advocacy and education
  • Assistance with understanding advance directives
  • Grief and loss counseling
  • Ensure and incontinent supplies for Pathways patients in financial need
  • Information about hospice and evaluations by hospice nurses by request
  • Help with the transition to hospice care if requested
