Our philosophy offers a holistic approach to end-of-life care. Physical pain and comfort needs are tended by the exceptional medical care provided by Shore Home Care Hospice, while the practical, emotional and spiritual needs of the hospice patient and family are addressed by Talbot Hospice staff and volunteers.

  • Shore Home Care Hospice provides the oversight of a Medical Director certified in palliative care,hospice registered nurses, social workers and home health aides
  • Talbot Hospice Foundation staff and volunteers provide practical, spiritual and emotional support completing the holistic approach of hospice care.

The Interdisciplinary Team of Hospice Care

  • The plan of care for each patient evolves under the direction of the Shore Home Care Medical Director, Dr. Lakshmi Vaidyanathan, who approves admissions to hospice.
  • Visiting hospice nurses are on-call 24 hours a day. Medical care comes to you in your home, which means no more appointments, emergency room visits or 911 calls.
  • Home health aides assist the patient with personal care needs for cleanliness, comfort and safety.
  • Social workers consult about their practical concerns, fears, future plans and arrangements.
  • The Bereavement Coordinator, Norma Trax-LCSW-C, is available to the patient and family for one-on-one support and counseling, and for family meetings. Following the loved one’s death,bereavement and grief support are offered to the surviving family members.
  • The Talbot Hospice visiting Reverend Jody Gunn, can bring comfort and resolution to many of the end-of-life spiritual issues we face. His services can be non-denominational or he can supplement or work in tandem with the care of one’s own clergy. Our Chaplain offers loving, unconditional support and comfort with respect for each individual’s own beliefs and traditions.
  • The Director of Family Services, Wendy VanNest, is available to help with arranging volunteers, equipment and supplies.
